Join to apply for the D2A Healthcare Support Worker role at Brunelcare. We have an exciting opportunity to join our dedicated Discharge to Assess (D2A) team, helping people regain independence in the comfort of their own homes and support a return to independent living.

Responsibilities
* Assess customers’ needs to identify areas where support is needed to promote independent living.
* Motivate customers to build their confidence so they can achieve the goals they have set and return to independent living.
* Support customers with tasks around the home and with personal care such as hygiene, toileting and dressing.
* Work a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.
About You / Qualifications
* QCF Level 2 in Health and Social Care (or equivalent).
* Passionate about supporting people to be as independent as possible.
* Full driving licence and access to your own vehicle to travel between clients.

About The Role
The D2A (Discharge to Assess) service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but may need extra support in their home.
Team & Contact
You’ll join our specialist D2A team covering Bridgwater, Highbridge, Burnham-On-Sea, Cheddar and surrounding areas. The team works with clients discharged from hospital to support recovery at home.
