As the post holder, you will deliver direct interventions to service users at a level of intensity according to need, including those who require an assertive approach and are difficult to engage. This will include ongoing assessment, diagnoses (including review and clarification), and treatment of mental and physical health needs.
You will provide medical input and leadership to the WWKD CMHT, with particular responsibility for patients from surgeries in Melksham and Trowbridge.
Your responsibilities also include:
1. Providing advice to members of the Multi-Disciplinary Teams in the WWKD CMHT and attending regular MDT meetings.
2. Assessment and treatment of outpatients in clinics.
3. Holding CPA meetings for outpatients.
4. Visiting patients in their own residence if clinically indicated.
5. Liaising with other parts of the service, GPs, and non-NHS organizations where applicable.
6. Coordinating with Intensive and Inpatient services to facilitate smooth handovers of clients between services.
7. Fulfilling the Responsible Clinician role for patients subject to Community Treatment Orders.
