Other areas include CHC Assessments, Asylum/NRPF, Transition, Safeguarding Experience - qualified Social Worker required; * Undertaking comprehensive social care assessments and reports, with detailed support plans, as appropriate. * Knowledge and understanding of Direct Payments using the strength-based approach. * Social Worker should have sound knowledge around the continuing health care process and procedure and the legislation that underpins this, including the Decision Support Tools. * Social worker will also need to communicate closely and sensitively with family members of service users.
