We are looking for a dynamic and enthusiastic pharmacist to join our band 7 rotational program, delivering best practice care to patients. We offer high quality education and training to develop pharmacists into accomplished practitioners in a supportive and team-orientated environment. The post includes the opportunity for postgraduate study, support to build your e-portfolio and the ability to experience all aspects of hospital pharmacy in a rotational program across Medicine, Surgery and Urgent Care. Our on-call service is currently 1 night in 24, with a backup on call service for additional support. Clinical mentors will support you in your role and there is also a regular forum organised by you and your peers. There is lots of opportunity to help shape the delivery of the clinical pharmacy service alongside clinical activity time for training, audits and development. Job Description and Responsibilities

Provision Of Clinical Pharmacy Services The post holder will:

* Provide clinical pharmacy services on rotation to the acute medical, surgical, critical care units, paediatrics and frailty medicine under the clinical supervision of Lead Specialist Pharmacists.
* Provide specialised pharmaceutical advice concerning the care of patients and undertake pharmaceutical risk management for the areas where duties are being carried out.
* Ensure compliance with medicines legislation, the Trust Medicines Policy, and medicines related national and local guidelines within the areas where duties are being carried out.
* Be accountable for own professional actions guided by SOPs, policies, national directives, standards and colleagues.
* Plan duties and prioritise workload according to patient need to complete tasks such as: analysing pharmaceutical care needs; identifying and correcting prescribing errors; advising prescribers on doses and treatments; communicating complex recommendations to staff; interviewing newly admitted patients to construct medication histories; reviewing information and discharge planning; constructing pharmaceutical care plans; monitoring and adjusting therapies as needed.
* Monitor TPN daily and assess discharge information requirements to prepare medicines information for patients.
* Assist with therapeutic drug monitoring, including initial assessments and dose adjustments.

Provision Of Information And Counselling The post holder will:

* Advise medical, nursing and technical staff on safe management and disposal of medicines; advise on storage, stability, compatibility and expiry issues.
* Provide information to consultants, GPs, and staff on medicines and therapy; counsel patients with complex needs.

Provision Of Professional Dispensary Services The post holder will be responsible for:

* On a rotational basis, checking prescriptions dispensed by Worcester/Alexandra Dispensaries for accuracy, appropriateness, legality and cost-effectiveness; ensure adherence to formulary and high-cost drug procedures.
* Question and challenge prescriptions when appropriate; ensure accurate dispensing and documentation of clinical trial material.
* Use dispensing systems and ePMA for processing prescriptions.

Managerial Responsibilities The post holder will:

* Provide day-to-day clinical supervision for pharmacy Foundation Trainee graduates, band 6 pharmacists, pharmacy technicians, assistants and students.

Financial Responsibilities The post-holder will:

* Help maintain the security of medicines and supplies; undertake regular checks of Controlled Drugs on hospital wards; advise wards on safe handling of medicines.

Education, Training And CPD The post holder will be responsible for:

* Participating in the Trust personal development review (PDR) and developing a yearly training plan (PDP) with the Clinical Pharmacy Team Lead; identifying training needs and completing objectives.

Other Operational Responsibilities The post holder will:

* Assist in the delivery of the pharmacy department’s dispensing, operational and clinical services; ensure accurate dispensing and preparation of medicines; participate in Weekend and Bank Holiday rotas; undertake emergency duties as required.

Qualifications Essential and Desirable

* Essential: Qualified pharmacist registered with the GPhC; postgraduate training in clinical pharmacy or equivalent; eligible for Independent Prescribing; meeting GPhC CPD standards.
* Desirable: Hospital pharmacy experience; Accredited Independent Prescriber; Health Assessments qualification; RPS Faculty membership; leadership/management qualification.

Experience Essential and Desirable

* Essential: Postgraduate experience delivering clinical services; experience influencing clinicians; supervising staff; contributing to reports, audits and guidelines; quality improvement.
* Desirable: Practicing prescriber; extended service as sole clinical practitioner; training nurses and doctors; leading a team.
