Our Community GPs have the opportunity to apply their experience, knowledge and interest in highly complex patient care in the patient's own home through working with the Team Up Home Visiting Service in North Hardwick and Bolsover PCN.
You will clinically lead a team of highly skilled and knowledgeable Advanced Clinicians to deliver complex holistic patient care. You will also support the Service Clinical Leads and Operational Lead in developing and applying service improvements.
You will also provide support to our trainees, helping them achieve their academic goals.
