An exciting opportunity has arisen for a band 5 Nurse Practitioner to join our acute inpatient team. You will provide a high level of care and support to people with complex care needs on long-term placements.


Main duties of the job

The post holder will work closely with multi-disciplinary teams to ensure suitable packages of care for service users. The post holder will work with the team to provide a person-centred approach.

Job responsibilities

1. Understand and follow good practice within the framework for the Mental Health Act, Mental Capacity Act, DoLs, and Care Programme Approach.
2. Maintain accurate records, both written and computerized, reflecting your level of clinical expertise, possibly involving specialized psychometric tools for assessment and evaluation.
3. Liaise with community-based services and resources such as Focus, housing, Police, and voluntary sector organizations to facilitate the delivery of the most appropriate care for service users, carers, and families.
4. Discuss risk management issues and plan care accordingly, advising others regarding management of care with senior practitioners.
5. Respond proactively to service user crises.
6. Follow clear care pathways, including methods to reduce the risk of further crises.
7. Work flexibly within a 24-hour rota covering early, late, and night shifts according to service user needs.
8. Provide telephone support to service users, carers, outside agencies, or clinicians requiring information, advice, or crisis intervention, making immediate, effective decisions impacting positive outcomes.
9. Undertake comprehensive CPA mental health assessments and develop/implement care packages in various settings.
10. Assist in maintaining safe custody of drugs according to trust policies and procedures.
11. Cover 7 days a week, 24 hours a day on a rota.


Person Specification


Qualifications

* RMN or awaiting registration
* Evidence of current registration
* Basic IT skills
* Evidence of ongoing professional development since qualification


Knowledge

* Respond to service users, carers, and significant others in a person-centered way
* Maintain a safe environment for all
* Work effectively as part of a team and communicate well with service users and professionals
* Assess individuals, formulate, implement, and evaluate care plans
* Participate in clinical supervision
* Understand the Mental Health Act, Care Programme Approach, Mental Capacity Act, and DoLs


Experience

* Experience working with people with mental health problems
* Experience working with older adults with mental health problems
