Overview
We have an exciting opportunity for a Diabetes Specialist Nurse to join our team at Shropshire Community Health Trust. The purpose of the role is to deliver a comprehensive, skilled and evidence-based Diabetes Service to meet the needs of people with diabetes. The role will provide leadership, management and development within the Diabetes Team while maintaining a clinical caseload within the specialist field. The post holder will be responsible for assessment of health needs and development and evaluation of standards of care. Responsibilities
* Assess, plan, implement and evaluate evidence-based nursing care for people with diabetes.
* Provide expert clinical care for patients and support other staff to develop their competencies in diabetes management.
* Teach and train formally to clinical staff and to patients.
* Collaborate across primary and community care and link with other specialists within the acute setting to implement the integrated care pathway for people with diabetes.

Person Specification
Qualifications — Essential
* Registered Nurse/RGN
* Evidence of post-registration CPD or experience in diabetes nursing
* Developed specialist knowledge through working with patients with chronic conditions including diabetes
* First Level Degree or equivalent
Experience — Essential
* Post-registration experience
* Evidence of nursing people with diabetes in an acute or community care setting
* Experience of providing training/education to a range of staff and patients
Knowledge — Essential
* Advanced communication, negotiation, decision making and organisational skills
* Ability to communicate complex and distressing information to patients and their carers in a sensitive and supportive way
* Ability to manage a complex caseload of high-risk patients
* IT skills and willingness to advance
