Social Worker, All Age Disability (AAD) Worcestershire Service working in the Young Adults Team (16 – 25) with specialist skills across Adult Services and Children’s Services. Children and Adults social worker with expertise and understanding of Children and Adult legislation. The AAD aims to make a difference to all children and young people aged 0 – 25 with SEN and additional needs. Experience of working in transitions in essential. The successful applicant will be joining a friendly and supportive team. You will have ongoing support in their role and regular supervision, and will also have a supported induction into the role. We cover all of Worcestershire, with some placements nationally, so any applicant would need the ability to drive. The All Age Disability Service The Young Adults Team team support young people with disabilities aged 16-25 in Preparation for Adulthood in areas of education/employment, independent living, community inclusion and health. The work is primarily with young people with Learning Disabilities, but also some young people with a physical disability or complex health needs, to transition from Children’s Services into their adult life. The work is very varied, and requires working in a multi-agency manner with Worcestershire Children Services, Adult Services, Special Educational Needs Department (SEND), health colleagues, schools and colleges to ensure young people get the whole range of options they deserve when moving into adulthood.
