We are excited to invite applications for the role of Mental Health Practitioner to join our dynamic and innovative team at Salford Living Well. As a community mental health service delivered in partnership with GMMH, Mind in Salford, Six Degrees, Wellbeing Matters, and START, we emphasise collaboration with service users and local communities.

We are seeking an experienced Mental Health Practitioner with previous experience of working with moderate to severe mental health challenges within a multidisciplinary team environment. Our broader team includes professionals from various backgrounds such as recovery workers, peer support workers (experts by experience), occupational therapists, social workers, nurses, pharmacists, doctors, and psychiatrists. Together, we aim to provide holistic support with a strong focus on both psychological and social wellbeing. Main duties of the job

This role offers a dynamic and rewarding experience, encompassing a variety of responsibilities including clinical work, staff support and supervision, training, and service development. You will have your own caseload, and your clinical duties will involve conducting initial conversations, developing care plans and formulations, delivering mental health interventions, and liaising with, or making referrals to other services. You will also be required to explore and manage safety or safeguarding concerns, with support from the wider team. Additionally, post holders will supervise other members of the team and provide support to the broader team and within your allocated neighbourhood. You will be included in the team hub rota, offering support to the wider team with any urgent enquiries, on your scheduled days.

7. Registered professional qualification
8. Willingness to participate in own continuing professional development within this specialist field
9. Experience of working within a multi disciplinary team
10. Experience of community working
11. Experience of working with individuals with complex mental health needs
12. Effectively able to manage own caseload
13. Ability to conduct first line assessments for individuals newly referred into the service, utilizing a biopsychosocial framework
14. Maintain confidential case records
15. Participate in multi-agency clinical meetings
16. Full understanding of strengths based working and trauma informed Care
17. A range of skills/Interventions that will meet the need of a diverse client group
18. Good knowledge and application of Risk formulation/management
19. Good knowledge of Mental Health Act and Mental Capacity Act and Care Act
20. Knowledge and understanding of the Clinical and Social care Governance standards
21. Ability to undertake visits on a daily basis to service users homes within caseload, clinical meetings at Trust and Non-Trust venues, and other community venues relating to your service user activity
22. Prepared to work flexibly to meet the needs of the service
23. Good IT knowledge and experience of case recording on systems

The Trust mandates all post holders who require a DBS for their role to subscribe to the DBS Update Service. You will be required to enrol for this service for a fee of £16 per year and maintain registration throughout your employment with GMMH.
