Deliver Advanced Clinical Interventions
Provide specialist harm reduction care (. BBV screening, Naloxone, wound care), undertake holistic health assessments, and make independent clinical decisions in community and outreach settings, including via the harm reduction van.
Promote Access to Healthcare
Identify unmet health needs and actively support individuals in accessing GPs, mental health, addiction, and dental services. Act as a health case manager in the short term for those with complex needs.
Ensure Safe, Responsive Practice
Maintain safety through robust risk assessments, lone working protocols, and accurate clinical documentation. Respond to safeguarding concerns and contribute to protection planning as needed.
Collaborate and Advocate
Work closely with partners across health and social care, attending multi-agency meetings and ensuring coordinated care and effective communication of treatment progress.
Support Prevention and Public Health Goals
Contribute to CGL’s Naloxone Strategy, deliver BBV interventions, and promote health improvement and disease prevention at every opportunity.
Shape and Improve the Service
Gather and act on patient feedback, support peer involvement, help develop local clinical policies, and contribute to service development and delivery planning.

Build therapeutic relationships grounded in compassion, trust, and dignity. Uphold the Charter of Rights for People Affected by Substance Use.
This is a hands-on, highly autonomous role for nurses who want to make a tangible difference in people’s lives — outside the clinic walls and directly within the communities that need us most.
About you:
Clinical experience working with vulnerable or marginalised groups
Ability to independently manage a caseload
A full UK driving licence (essential for community-based outreach)
Ideally, you’ll also bring:
Experience making autonomous clinical decisions
Additional qualifications in minor illness, injury or chronic disease management
First-hand experience delivering nurse-led outreach in community settings
You’ll also need:
A clear understanding of safeguarding practices
A strong commitment to equality, diversity and inclusion
Confidence working both independently and as part of a close-knit team
