The Role
A vacancy has become available for a full time, permanent Integrated Discharge Facilitator within the Integrated Discharge Hub based at University Hospital Monklands.
In this key role, you will:
* Have responsibility for early assessment of care needs to support effective discharge from hospital for patients with complex discharge planning needs. The post holder will ensure there is effective communication between all relevant health and social care services to ensure high quality patient‑centred discharge planning and carer support.
* Take a role in the education of clinical teams and partnership colleagues, providing specialist advice on the availability of appropriate community services and referral processes which enable people to live and die in their own homes.
* Be responsible for generating and maintaining a range of data regarding the status and numbers of people currently receiving care in hospital who are clinically ready for discharge.
* Contribute to service developments and will be responsible for evaluating, implementing and monitoring discharge planning with a wide range of colleagues and across NHS Lanarkshire as required.
* Contribute towards service developments and innovations within the service.
* Be responsible for evaluating, implementing and monitoring discharge planning standards, giving advice and education as appropriate within the Hospital and pan Lanarkshire, where appropriate.
What You'll Bring:
* Registered Nurse or AHP with valid NMC / HCPC Registration.
* Educated to/working towards/operating at Degree Level.
* Post graduate expertise within speciality / area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently.
* Comprehensive knowledge of clinical guidelines and standards within the speciality / area of practice.
* Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within speciality / area of practice.
* Management qualification or equivalent gained through experience and continuous professional development.
* Further education/evidence of Continuous Professional Development in area of speciality including study days/courses/post graduate qualification.
* The post holder will possess excellent team‑working/leadership skills and have the ability to motivate others and work using own initiative.
* Excellent listening, communication and interpersonal skills.
* Effective time management skills.
* Working knowledge of basic information technology.
From 1 April 2026, the Agenda for Change full‑time working week will reduce from 37 hours to 36 hours. Part‑time working hours will be reduced on a pro‑rated basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
