About Us
We are recruiting for a Nurse Practitioner to join our whānau-focused Hauora Heretaunga Medical practice.
At Hauora Heretaunga, our Cornerstone Accredited teaching practice focuses on delivering exceptional, evidence-based primary care services to our diverse patient population of over 6,000 whānau. Our model of care is similar to the Health Care Homes framework. We prioritise improving equity and health outcomes of our local community, with the majority of our clients being Māori and Pasifika.
TToH is a whānau-focused health and social services organisation driven by our kaupapa Māori values. For 40 years, we have been dedicated to enhancing health equity and improving Māori health across the Hawkes Bay region. As one of NZ's largest kaupapa Māori organisations, our holistic approach integrates health, social, education, and community development services, all designed to empower whānau to better health and social outcomes.
Role Overview
As a Nurse Practitioner you will be empowered to make decisions and recommendations to facilitate high‑quality care and coordination for whānau with complex health needs. Your work will involve collaborating across practitioners and health and social services to ensure seamless transitions of care, ultimately improving health outcomes.
Key Responsibilities
* Patient assessments and treatment
* Provide clinical leadership and individual solutions
* Coordinate patient care with our GPs, other services and health providers as needed
* Ensure continuum of care
* Keep current with best practice to enhance professional practice
Qualifications
* Registered Nurse Practitioner with current annual practicing certificate.
* Primary Care Nursing experience is ideal with experience in Cardiac, Cancer, Diabetes, Asthma and Women’s Health
* In‑depth understanding of health equity in Aotearoa and commitment to Te Tiriti o Waitangi.
* Exceptional communication, organisational and critical appraisal skills.
* Ability to independently manage complex whānau and demonstrate innovation in care delivery.
* Strong ability to build trust with whānau, promoting health initiatives and supporting change.
The Role
Ideally 32 hours per week [0.8 FTE]. Our clinic is open 8.30 am to 5.00 pm Monday to Friday. No weekends or shift rosters.
