As a practice nurse at Cambridge Avenue & Messingham Medical Centre, you will play a vital role in delivering primary care services to our diverse patient population. You will work autonomously while being fully supported by a well-established team. The practice prioritises patient care and fosters a supportive, team-oriented environment.
We value flexibility. While we are looking for approximately 30 hours a week, full-time hours for the right candidate would also be considered. We ask for some adaptability to accommodate training, holiday cover, and occasional colleague absences.
Main duties of the job
The practice nurse will work as part of an excellent administrative and multidisciplinary clinical team to provide high-quality nursing care to patients. They will play a key role in patient education and supporting self-care initiatives.
Providing and evaluating a range of health interventions to meet patients' health and social care needs.
Promoting health and well-being through individual, person-centred plans for patients with or at risk of long-term conditions.
Managing and treating long-term conditions in line with national and local policies and practice needs.
Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice and protocols, and supporting patients in adhering to prescribed treatments.
Prioritising health issues, intervening appropriately to assist patients in complex, urgent, or emergency situations, including initiating effective emergency care; supporting and managing health needs of women during family planning and cervical screening consultations.
Implementing and participating in vaccination and immunisation programmes for both adults and children, including travel vaccinations.
Providing opportunistic wound care for patients.
Conducting hypertension reviews and management; running clinics and reviews for asthma, COPD, and diabetes.
Carrying out new patient and health checks.
