FLO: * Support colleagues to create and record a care and support plan * Responsible for ending a care and support plan. * Support colleagues to create and record an integrated assessment/CTP assessment * Responsible for systems administration e.g. adding a new user, removing duplicates, troubleshooting, adding police notification etc. * Responsible for creating a new person and closing a case down This will include use of the application Tell us Once, CHC and FNC correspondence – closing a case down does not include Adult Disability Services as the Senior Practitioner is responsible for this task) * Responsible for the administration of FLO allocations * Represent the team at the Flo Systems Group CEQIP: (Integrated Services Teams) * Support colleagues with satellite store equipment ordering and stock control * Support colleagues with the processing of equipment returns EPEX: (MHCT) * Responsible for inputting onto EPEX to ensure CTP compliance/cross referencing Systems * Support colleagues to work effectively using corporate or service specific digital systems
