Triage Practitioner

This role will provide a single point of access for all referrals into the Memory Service within Older Adults Mental Health.
The role will ensure all referrals are screened in line with the All Wales Mental Health Triage Scale and categorised appropriately to manage risk.
It requires liaison with multi-agencies to establish comprehensive assessments of referral information, employ analytic and professional judgement when reviewing individuals or care partners, and discuss sensitive information with care partners if significant risk or lack of capacity is identified.
Main duties of the job
The role will collate and analyse statistical data from the current database to facilitate triage, making autonomous clinical judgments to prioritise and allocate referrals to the appropriate professional within the service or suggest alternatives to the referrer.
It will review all referrals to identify gaps, strengthening existing joint partnership systems for early and appropriate referrals.
In line with the All Wales Dementia Care Pathway of Standards, the role supports the administration of collating statistics and the development of a dementia register.
The role works as part of a multidisciplinary team, supporting colleagues in the community to ensure high quality services for older adults with mental health needs, collaborating with local authorities, third‑party sectors, and other disciplines within CTMUHB.
It includes supervision responsibilities for staff within the team.
The ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply.

Person Specification
Knowledge
* Wide knowledge of mental health problems in community, care homes, and inpatient settings
* Understanding and experience of multi‑agency and multi‑disciplinary working
* Good working knowledge of the mental health needs of older adults and their care partners
* Comprehensive knowledge of the Mental Health Act 1983, Mental Capacity Act 2005, Mental Health (Wales) Measure 2010, and All Wales Dementia Care Pathway of Standards
* Possession of a postgraduate qualification relevant to mental health, or evidence of working to the same
Registration
* Registered mental health practitioner (e.g., social worker, occupational therapist, psychologist)
Skills and Experience
* Adaptable, self‑motivated using initiative, clinical judgement, and problem‑solving skills
* Experience in risk assessment and risk formulation
Experience
* Experience at Band 5 or above within older adults mental health services
* Experience working with independent sectors
* Experience in risk assessment and risk formulation
Additional Skills

* Good computer literacy and data entry ability
