Overview
The Advanced Nurse/Clinical Practitioner (ANP/ACP) is competent in practice using expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients. The post holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area. The role provides an opportunity for patients to avoid unnecessary referrals to a GP, enabling timely care and reducing delays in treatment.
Responsibilities
* Manage patients presenting with a range of acute and chronic medical conditions.
* Be an extended and supplementary prescriber, adhering to guidance. Support patients in the use of prescribed medicines or over‑the‑counter medicines within own scope of practice, reviewing annually as required.
* Contribute to practice/PCN targets (QOF, etc.) complying with local and regional guidance.
* Identify, manage, and support patients at risk of developing long‑term conditions; request pathology services as necessary; process and interpret pathology and other test results; maintain accurate clinical records in line with legislation.
* Support the team in dealing with clinical emergencies and safeguarding matters in accordance with local and national policies.
* Practise in compliance with professional conduct and within the scope of practice; be responsible and accountable for decisions and actions; understand limits of competence.
* Engage in professional judgement, reflective practice, and seek help as needed; work in partnership with individuals, families and carers; communicate effectively to support decision‑making and care planning.
* Use expertise and clinical reasoning to inform decision making; manage risk and act as a professional role model for the organisation.
* Participate in peer review and seek feedback to drive service improvements; collaborate with individuals, families, carers, communities and colleagues.
* Develop a personal area of specialist interest and participate in local initiatives to enhance service delivery and patient care.
