Community Specialist Palliative Care Nurse (North & East) – Cornwall Partnership NHS Foundation Trust. This role is within the NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly.
Main duties of the job
* Provide specialist palliative care within the community, working with the multidisciplinary team and other specialist palliative care providers.
* Manage a geographically defined caseload and ensure high-quality symptom assessment, psychological support and specialist advice for patients with complex palliative needs.
* Use recognised referral guidelines to undertake agreed levels of intervention.
* Demonstrate clinical decision-making, monitor and improve standards of care through supervision of practice, clinical audit, research, teaching and support to professional colleagues.
* Participate in a rota providing a seven-day service.

Education/Qualifications and Relevant Experience (Essential)
* Current professional registration - NMC
* Professional qualification - RGN - level 1
* Degree level study in relevant subject
* Evidence of continuing professional development and post-graduate level specialist training
* Evidence of teaching experience
Desirable criteria
* Recordable teaching qualifications
* Independent nonmedical prescriber
* Counselling qualification
Skills and Aptitude (Essential)
* Extensive post-registration experience
* Knowledge of priorities of care of the dying person and gold standard framework
* Proven palliative or oncology experience
Desirable criteria
* Experience of working in primary care
Knowledge and Abilities (Essential)
* Ability to evaluate research and apply evidence-based practice
* Knowledge of clinical governance and ability to apply practice
* Ability to work autonomously and within primary health care teams and multi-professional teams
