Clinical Lead – Princeway Community Care Team, Frodsham
We are looking to recruit an enthusiastic Clinical Lead to join the Princeway Community Care Team in the Frodsham area. You will be working within a multidisciplinary team that includes Team Manager, Enhanced Clinical Practitioner, Clinical Leads, Community Nurses, AHPs and other specialist professionals.
This post is part‑time, 22.5 hours per week. The service runs between 8am and 8pm, 7 days a week. You will be expected to cover the rota until 8pm when required.
Shortlisting commences 21 December 2025, with proposed interviews on 31 December 2025.
Key Responsibilities
* Provide a highly visible clinical profile with leadership and clinical oversight of a designated community caseload
* Work collaboratively with wider health, social care, and third‑sector partner agencies to meet patient needs
* Ensure patients receive the right care, by the right person at the right time and place
* Place the patient at the centre of all decisions, reinforcing a streamlined journey with minimal avoidable delays and emphasis on independence and quality of care
* Provide direction within the caseload in consideration of current health and social care agendas and local initiatives
* Be aware of local public health community profiles and how they influence required care for the population
* Understand the skills required from the Community Care Team according to community profiles
* Focus on the delivery of person‑centred care
* Provide effective, evidence‑based care to a defined population through case‑load management and monitoring of care standards
* Provide clinical leadership
