CHC Nurse Assessor - Registered General Nurse (RGN)

An exciting opportunity has arisen to join Coventry and Warwickshire Integrated Care Board to work in our Continuing Healthcare Team within South Warwickshire. The service assesses patients for eligibility and secures placements for those funded by CHC, S117, NON CHC, and D2A across South Warwickshire.

You will join a team that has transformed the operational service to meet NHSE Quality Targets, discharge patients within 48hrs from an acute trust, and has improved patient experience while reducing complaints by 80%. Coventry and Warwickshire Integrated Care Board is seeking a Clinical Assessor - RGN to join their Clinical Assessment Placement Team, responsible for case management: being the single point of contact for patients and their families, supporting them through the assessment process. You will also collaborate with internal and external teams to secure appropriate placements and packages of care. Additionally, you will undertake regular care and eligibility reviews with local authority partners.


Main duties of the job

* Facilitate multi-disciplinary assessments and patient reviews in line with policies and procedures, including face-to-face and virtual assessments.
* Support the Complex Case Manager/Clinical Commissioning Manager and the wider team to plan and manage work effectively, ensuring successful delivery.
* Manage cases as directed by the Management Team.
* Case manage a clinical caseload end-to-end, resolving issues related to funding, placements, invoices, and complaints.
* Ensure Continuing Healthcare guidance is followed to maximize patient safety.
* Support organizational development in personalisation and pathway delivery for Personal Health Budgets within NHS Continuing Healthcare.
* Identify and support arrangements for services meeting the needs of individuals qualifying for NHS Continuing Healthcare and joint care packages.
* Investigate individual eligibility for NHS Continuing Healthcare following challenges or disputes.
* Act as Duty Clinician as required.

Job responsibilities

* Maintain up-to-date data systems in line with policies, ensuring compliance with the Data Protection Act 1998 and confidentiality standards.
* Participate in ongoing professional development and clinical supervision per NMC guidelines, maintaining revalidation.
* Provide cover for other Nurse Practitioners as needed.
* Maintain professional and appropriate communication.
* Support the Clinical Assessment & Placement Team to achieve team goals and objectives.
* Build and maintain relationships with internal and external partners/stakeholders.
* Communicate risks, issues, and dependencies through briefings and reports.
* Work closely with care providers involved in service delivery.
* Participate in relevant meetings, providing reports and advice.
* Ensure data accuracy and monitor information processing.
* Provide specialist advice on functional and information matters related to CHC.
* Deliver specialist services within your skillset.
* Accurately record information and support individuals regarding NHS Continuing Healthcare processes.
* Enhance management information and reporting processes.
* Contribute to performance, governance, and improvement strategies.
* Achieve objectives and performance targets, prioritizing tasks within deadlines.
* Support training and induction of new staff.
* Undertake research and development as directed.
* Perform other duties as assigned by line management, including assessments for patients with health funding, joint funding, or ABI.
* Support dispute management, transitional bed processes, SEND, and Safeguarding.
* Work effectively in a busy environment, demonstrating adaptability and professionalism.

Mobility

Employees may be required to work at various sites within the organization, subject to consultation, due to service developments and restructuring. Cooperation with organizational changes is expected throughout your contract.

This is not a home working/remote working only position.

Person Specification


Education / Training / Qualifications

* NMC registration as an adult nurse
* Full driving license/own transport
* Ability to travel and work in various settings
* Ability to work independently and organize workload under tight, changing deadlines
* Experience/knowledge of CHC
* Understanding of MCA, capacity, and consent
* Skills in data input, Microsoft Office, and keyboard use
* Effective communication and report-writing skills
