Detailed job description and main responsibilities
* Responsible for ensuring all requests for Access to Health Records are dealt with in an efficient and timely manner within set timescales.
* Ensure that all processes are compliant with relevant legislation and guidelines, particularly the Data (Use and Access) Act 2025, General Data Protection Regulation May 2018, Access to Health Records Act 1990 and NHS Confidentiality Code of Practice. Respond to requests for access to patient information from external and internal sources, prioritising requests and ensuring they are dealt with effectively within appropriate timescales as required by relevant legislations and codes of practice.

* Ensure all telephone enquiries are dealt with in an efficient and confidential manner.
* Responsible for processing all associated documentation regarding appropriateness of requests, photocopying, consents and identification checks ensuring that processes are consistent, transparent and legally compliant.
* Use the trust's patient administration systems to locate and track health records for the purposes of administrating requests for access to patient health records.
* Responsible for ensuring mechanisms are in place to retrieve records, copy records and to liaise with multi-disciplinary staff where the need arises regarding disclosure.
* Follow appropriate Trust procedures for obtaining authorisation for the release of patient information, either through relevant clinician, or Health Records Manager/ Information Governance Manager/Caldicott Guardian.
* Responsible for accurately recording all requests onto the Trust Subject Access Database, in a timely manner.
