Job Summary
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Main duties of the job include undertaking duties governed by the contract and Direct and Local Enhanced Services, comprising a variety of duties such as face-to-face surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, and dealing with queries, paperwork, and correspondence in a timely fashion.

Primary Care Networks (PCNs), supported by Dacorum Healthcare Providers Limited (DHPL), are developing new and exciting additional roles under the 2020 scheme to support GPs and clinical teams at enhancing patient services in the local community.
There is a wide socio-economic and demographic served within this area, which will provide an interesting and varied case mix. The role will contribute to improving the quality of patient care across the network of practices.
Job Description
Clinical Responsibilities include:
* Undertaking duties governed by the contract and Direct and Local Enhanced Services.
* Making professional, autonomous decisions in relation to presenting problems.
* Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
* Screening patients for disease risk factors and early signs of illness.
* Developing care plans for health in consultation with patients and in line with current practice disease management protocols.
* Providing counseling and health education.
* Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
* Recording clear and contemporaneous consultation notes to agreed standards.
* Collecting data for audit purposes.
Person Specification
Essential Qualifications:
* Full GMC registration.
* Completion of Certificate of Training (CCT) in GP dated within the last 2 years.
* No substantive post held (i.e., salaried role) after CCT date.
* Performers List registered.
Desirable Qualifications:
* Experience of working in NHS General Practice.
* Keeping up with CPD and good knowledge of current guidelines.
* Understanding of QoF, Medicines Management, and the general working of GP practices.
* Understanding of EMIS Systems.
* Excellent communicator and team player.
