About the role
Full‑time Adult Social Care Practitioner position in the Cotswold Locality Team, based in Cirencester. The role involves undertaking assessments and developing support plans for people with physical disabilities or other complex needs, working closely with families, carers, and agencies across Gloucestershire to promote independence, enable individuals to remain in their own homes, and reduce hospital admission or long‑term care.
Responsibilities
Perform health and social care assessments, manage a caseload of people experiencing a range of physical disabilities, mental health difficulties and learning difficulties, and collaborate with multidisciplinary teams to provide integrated care.
Qualifications & Experience
* Relevant qualification equivalent to NVQ Level 3 or equivalent Health and Social Care experience.
* Training related to Health and Social Care assessments.
* Experience managing a caseload, including working with people with physical disabilities, medical, mental health and learning difficulties.
* Demonstrated good interpersonal skills and verbal/write communication.
* Valid UK driving licence and ability to travel across the rural Cotswolds area.
