Liaison Nurse - Liaison Psychiatry for Older People

This dynamic liaison team work with adults over the age of 65 with the aim to facilitate earlier discharge from hospital. This is achieved by providing timely assessments, advice and interventions. The teams' working practice in underpinned by involving patients and carers in decision-making while working in parallel with medical and surgical teams.
The role of the successful candidate will include the managing a small caseload of patients. This will include completing assessments, advising, supporting junior members of the team particularly highly complex patients. The ability to delivering specialist teaching/training of UHB staff will also be a key part of the role.
The post holder will work as part of a team to deliver a high standard of care which is least restrictive, goal focused and patient centred. The team operates across all hospital sites within the UHB.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The post holder will work closely with the Team Leader/Ward Sister/Charge Nurse, Deputy Team Leader/Deputy Ward Sister/Charge Nurse and multi-disciplinary team members in monitoring and maintaining excellent clinical standards of nursing practice and staff development.
The post holder will be responsible for the continuing professional development and overall performance of staff by acting as a mentor and preceptor.
Where applicable, the post holder will take charge of the team/ward/department on a regular basis.
The post holder is expected to carry out all relevant and specialised forms of care without direct supervision and is required to demonstrate procedures, supervise registered and/or unregistered staff whilst maintaining a safe working environment.
The post holder will be flexible in the approach to the clinical area and provide cover for any area specified.
