Brentford Group Practice is a well-established GP practice/list size 8200, CQC rated as good with high QOF achievement and we use System One. We are looking for an enthusiastic and dedicated Practice Nurse for 15 hours a week over 2 days. We are looking for someone who shares our commitment and passion for providing high-quality patient care and who also has the desire to attain their own individual career goals.


Main duties of the job

The post holder will provide care for the presenting patient, treatment, chronic disease management and evaluation of care. As well as providing the full range of primary care services to our patients, including immunisation for adults and children, cervical cytology, wound management, Asthma and COPD reviewS, Diabetes review, Swabs, Mental Health Review, Travel Vaccination, Diabetic Checks Health checks, INR, Infection control management/audits etc.

An individual who can work collaboratively with the general practice team to meet the needs of the patients, ensuring that practice policies and procedures are followed.

To be considered for this exciting role you must be a NMC UK registered nurse with significant post-registration experience as well as the Right To Work in the UK

In return we offer market leading salary scales 25 days annual leave plus bank holidays pro rata.

Annual appraisals with formal clinical supervision

About us

Our list size is 8200 with an interesting mixed demographic. We are active within PCN and our PCN staff consists of pharmacists, first contact physio, a social prescriber, a mental health advisor, a paramedic and a dietician. We are rated Good by CQC and we use system one.

We value best practices, support one another and focus on professional development and flexibility for our staff. We have several meetings including clinical and practice meetings and we encourage progress in leadership and special interests.

We are within a purpose-built health center with good facilities including spacious and air-conditioned clinical rooms including free staff car and bicycle parking. We are located within easy access to Brentford station, Boston Manor underground station, easy link to London, facilities etc.


Job responsibilities

The post holder will provide care for the presenting patient, treatment, chronic disease management and evaluation of care. As well as providing the full range of primary care to our patients including immunisation for adults and children, cervical cytology, wound management, Asthma review, Diabetes review, Swabs, Mental Health Review, Health checks, INR, Infection control management/audits etc.

An individual who can work collaboratively with the general practice team to meet the needs of the patients, ensuring that practice policies and procedures are followed.

Asrequired, to provide routine nursing care to patients in accordance withclinical-based evidence, NICE and the NSF

.To oversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required

To identify, manage and support patients atrisk of developing long-term conditions, preventing adverse effects on patients' health

Toimplement vaccination programmes for adults and children including forhousebound patients

Toprovide treatment of minor injuries, dressing, removal of sutures

Toundertake Cardiovascular Screening in patients aged 40-74 opportunistically.Provide lifestyle advice and education, and refer patients where appropriate tohealthy lifestyle programmes.

Maintainhealth promotion templates updating info on CHD, alcohol, smoking, and obesity,and ensure appropriate registers are maintained for call and recall ofpatients.

Tocounsel men and women seeking sexual health advice and initiate sexual healthscreening in line with the local public health contract

Toprovide contraception advice service, starting people on COCP, POP and Depo inconsultation with the doctors and in accordance with PGD.

Takingof cervical smears. Maintaining a smear recall system and notifying patients ofresults. To set up regular relevant audits and ensure they are carried out atthe appropriate intervals.

Providean Oral Anticoagulation therapy service for patients, using INR star.

Responsiblefor maintaining adequate stock of dressings, drugs, specimen bottles, swabs,instruments, diagnostic agents, utensils and leaflets. Responsible for ensuringall consultation rooms are stocked and clinical waste is removed on a regularbasis.

Tohave overall responsibility for maintaining the cold chain, including managingthe fridges and ensuring staff understand their roles & responsibilities.

Responsible for maintaining clinical equipment, ensuring there are cleaning& maintenance schedules in place in line with the manufacturersrecommendations.

Responsiblefor maintaining the treatment rooms in a clean and tidy manner and correctdisposal of clinical waste.

Tomaintain accurate clinical records in conjunction with extant legislation

Toensure read codes are used effectively by all members of the nursing team

Toensure the appropriate maintenance and use of chronic disease registers

Toprovide chaperoning and assisting patients where appropriate who are beingexamined by another clinician. Ensure compliance with the practice chaperonepolicy.

Tosupport the team in dealing with clinical emergencies

Torecognise, assess and refer patients presenting with mental health needs

Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)

Todelegate clinical responsibilities appropriately (ensuring safe practice andthe task is within the scope of practice of the individual)

Toattend regular clinical and practice meetings

Tosupport the clinical team with all safeguarding matters, in accordance withlocal and national policies and attend safeguarding meetings

Todeliver opportunistic health promotion where appropriate

Anyother duties as may be determined from time to time relating to the range ofactivities described above.


Person Specification


Qualifications

* - UK Nursing Qualification
* - Registered with the NMC
* Registered Nurse with Nursing and Midwifery Council Qualified Independent Nurse Prescriber on the NMC register Meets the standards for registered Nurse.
* Experience:
* GP Practice: 1 year (preferred)


Experience

* Experience of working within a nursing team
* Experience in dealing with vulnerable patients
* Experience of working in primary care
* Experience of working in a GP practice
