Job overview

The post holder will manage a caseload/client group providing highly specialist expert nursing advice, information and support to patients with lung cancer and their families at all stages of their disease and in particular whilst they are being cared for at South Tees NHS Hospitals Foundation Trust thus ensuring a smooth transition of care from primary treatment through to radiotherapy/chemotherapy and into the community, to discharge or death, thereby establishing links between the acute and primary care group, supporting both patient and carers.


The post holder will liaise with services within South Tees Hospitals NHS Trust and with other key partners such as Macmillan Cancer Support, Northern Cancer Alliance, Clinical Commissioning Groups, Primary Care Trusts, Social Services and Hospices to enable and ensure quality service provision for all patients under their care. They will provide assistance, support and health promotion advice to other staff, whilst improving clinical practice, enhancing service provision and streamlining patient focused care.

The post holder will be an active core member of the multi-disciplinary team and network with other professionals, locally and nationally for the purpose of developing practice, research and audit, development of standardised patient information, and clinical supervision.

Main duties of the job

The post holder will manage a clinical caseload and client group within the lung cancer setting from diagnosis throughout the treatment trajectory to death or discharge.
· Plan specialist nursing provision and interventions as necessary, referring patients to other members of the lung cancer MDT/other relevant teams as appropriate thus promoting continuity of care.
· Participate in lung cancer MDT discussing patients as necessary to facilitate the co-ordination of patient care.
· Co-ordinate the patients care trajectory, liaising with all members of the MDT to ensure appropriate and timely investigation, treatment and support.
· Conduct nurse led face to face and telephone follow-up clinics for identified cohort of patients
· Provide ongoing specialist support service for both patients and carers.

· Act as a key worker for patients with lung cancer ensuring that all patients, carers and appropriate community colleagues GP/district nursing and support services are well informed about the patients disease and its management providing accurate and timely information.
· Provide clinical expertise to primary/secondary care staff regarding the care of patients in this specialist group, acting as a resource for health care professionals, patients and carers,

Qualifications

Essential criteria

1. NMC level 1 registration
2. Specialist qualification in cancer care/palliative care nursing/respiratory medicine
3. Physical assessment & disease management skills course (level 7/level 6)
4. Teaching Qualification ENB 998 or C&G 730 or equivalent
5. Advanced communications skills course or willing to undertake
6. Physical assessment & disease management skills course (level 7/level 6)
7. Hold a record of continuous professional and academic development.

Desirable criteria

8. Independent non-medical prescribing
9. Masters degree or if working towards an expected completion date or willingness to study at advanced level/Masters
10. Recognised counselling course or certificate

Knowledge

Essential criteria

11. Knowledge of strategic issues which influence management of the care of patients diagnosed with a lung cancer at a local, regional and national level
12. Knowledge of audit and research processes and its application
13. Knowledge of the Clinical Governance agenda and NHS Plan
14. Knowledge about the development of the specialist nurse role in a modern NHS
15. Understanding of the need for strong commitment to high standards of patient care and the active involvement of users in the development of the service
16. Ability to work in a complex environment across the whole of the lung cancer pathway.
17. Advanced interpersonal/communication skills including self-awareness and emotional intelligence.
18. Ability to lead and develop staff/service as part of a team to optimise resources
19. Leadership and influencing skills

20. Expert level of knowledge of lung cancer management and care of those undergoing lung cancer treatments

Experience

Essential criteria

21. Extensive post registration experience, with experience of working at a senior level.
22. Recent experience in cancer/ palliative care
23. Experience in supporting and managing for those with cancer
24. Experience of input into audit
25. Understanding and experience of service improvement
26. Current teaching experience
27. Experience of working within a busy clinical environment and within multidisciplinary teams.
28. Evidence of further study within field of expertise or a willingness to undertake further training where appropriate

Desirable criteria

29. Experience in providing care for patients with lung cancer or other long term conditions
