Job Description:
1. Within the framework of Community Care legislation and professional guidance, undertake outcome-focused assessments and care management tasks for older people and their carers.
2. Work will be undertaken with access to regular supervision and advice. It does not require decisions involving legal powers or duties leading to the loss of liberty of service users.
3. Identify and manage risks to service users and carers within policy guidelines.
4. Design tailored service packages to meet assessed needs, including aids to daily living, aligning with departmental policies and priorities, and agreeing objectives with individuals.
5. Negotiate effective ways to achieve identified objectives, liaise with other professionals and agencies, and incorporate them into individual care plans. Ensure care plans are shared with service users and carers as appropriate, and actions are properly recorded.
6. Maintain close links and liaise with other department staff and external agencies (e.g., Housing, Health, voluntary, and independent sectors) to ensure an integrated service approach.
7. Oversee monitoring, reviewing, and reassessment of care packages. Address concerns about service provision (both in-house and external) and ensure individual needs are met. Contribute to reviews, including multidisciplinary arrangements, and prepare necessary reports.
8. Agree on variations of care packages within set parameters.
9. Be aware of the ethnic and cultural needs of the local population and develop culturally sensitive care packages, seeking advice as needed.
