About the Role Facilitating safe and timely hospital discharges for adults Carrying out Care Act 2014 assessments and reviews Coordinating care and support packages tailored to individual needs Working collaboratively with NHS colleagues, families and partner agencies Promoting independence, wellbeing and choice for service users Managing cases effectively within a hospital setting Why This Role?
