Overview
The Home-Visiting Advanced Nurse Practitioner/Enhanced Practice Nurse role exists to support the delivery of the Federation Home-Visiting service, working alongside the Paramedic Practitioners and supported by the Service Delivery Manager, Federation HR and Admin Officers, Directors and Clinical Directors. The ANP/EP N is a professionally autonomous practitioner providing home visits to patients registered with one of our sixteen practices and is part of a clinical team. The post holder will assist in developing protocols and guidelines within the service and work with clinical colleagues to ensure the highest standards of care for all patients.
Responsibilities
* Deliver high-quality, autonomous care across clinical, professional, and managerial domains, including conducting home visits using advanced assessment, diagnostic, and prescribing skills to manage acute and chronic conditions.
* Arrange and interpret investigations, develop care plans, and promote health and well-being.
* Prioritise patient health needs, manage mental and community health presentations, and liaise with external agencies to support vulnerable patients.
* Ensure safeguarding, accurate clinical coding, and documentation in line with professional standards.
* Adhere to NMC standards, maintain confidentiality, and engage in continuous learning. Prescribing is carried out in line with legislation and local guidelines.
* Proficiency in clinical systems (e.g., SystmOne and WebV) to support safe, effective care.
* Manage workload, collaborate with colleagues, and contribute to service development.
* Participate in meetings, audits, and quality improvement to align with best practice and patient-centred care.
Qualifications and Requirements
* Professional registration with the Nursing and Midwifery Council (NMC) and current practising status.
* Advanced nursing/clinical practitioner competencies, including autonomous decision-making and ability to manage undiagnosed problems.
* Prescribing authority in line with legislation and local guidelines.
* Proficiency in relevant clinical systems (e.g., SystmOne and WebV).
* Commitment to safeguarding, confidentiality, and continuous professional development.
