Community Care Worker - Adults Services
£19.85 per hour
Derbyshire
37 hours p/w, contract until March 2026


About the role

Social Care Practitioner for Occupational Therapy. Working in the High Peak and Derbyshire Dales areas. Face-to-face visits in the community are an integral element of this role. The purpose is to support a backlog of non-complex Occupational Therapy waiting list assessments. Candidates do not need to be a qualified Occupational Therapist but should have a Level 3 Diploma in Adult Care.

The post holder will have strong assessment skills, work closely with the person’s assets, an equipment provider, and Housing and Health departments to help people remain independent at home through small aids and minor and major adaptations. The role requires a proven track record of working in a Local Authority environment and experience of Occupational Therapy interventions, including equipment solutions and housing adaptations, in partnership with housing providers.

You will be supported by an experienced management team, a wider network of Social Care Practitioners, Occupational Therapists, Team Coordinators, and a dedicated Occupational Therapy service manager and senior practitioners. A strength-based approach is expected to deliver a proportionate, personalised, safe and sustainable response for local adults, their carers and families. A positive approach to risk taking, accurate record keeping, personal resilience, and the ability to contribute to inter-agency and multi-disciplinary work are key.

Working days: Full time.


What you’ll bring

* Knowledge and/or experience of working in a person-centred, holistic and strengths-based way.
* Knowledge and/or experience in assessing a person’s activities of daily living.
* Knowledge and/or experience of physical, learning disabilities and/or mental health conditions and their impact on daily living.
* Knowledge and/or experience of equipment and adaptation solutions.
* Ability to manage and prioritise casework.
* Strong communication skills and ability to form positive relationships with a wide range of partners, professionals and agencies.
* Good IT, time management skills and high motivation. Experience of Mosaic recording system is desirable.
* Understanding of statutory legislation underpinning Occupational Therapy assessment and intervention in adult social care and health.
* Reflective practitioner with commitment to supervision and development within codes of conduct.
* Knowledge and/or experience of safeguarding principles and how they are embedded in practice.
* Ability to travel for home visits.

Duties & Responsibilities

Work with clients and their families/carers to identify and mobilise strengths, networks, and communities to maximise independence, self-care, social and occupational opportunities as appropriate. Work with other professionals including social workers and occupational therapists, referring on as necessary. Assess and review individual needs and arrange or refer services; including prescribing and delivering simple equipment and non-complex housing adaptations as needed. Review work and caseload with senior practitioner/service manager at regular intervals, including case closures or transfers. Undertake holistic assessments for individuals with less complex needs to identify eligible outcomes and support viable care and support plans in line with legislation. Share information with clients about resources and link people to local community resources/activities. Liaise with voluntary bodies, residential and day establishments, and other agencies. Ensure safeguarding of vulnerable adults.
