About the Role
This post is only open to current employees of the Trust. An exciting opportunity has arisen for an existing or newly qualified band 5 to obtain a role in the Crisis and Home Treatment Team. We will support the successful candidate through induction and completion of preceptorship. The team provides crisis assessment and home treatment to patients in acute mental health crisis within the least restrictive environment, works closely with inpatient services for early planned discharge, manages the Hatton Suite for those detained under S136 of the MHA, and offers an overnight liaison service to Wye Valley Trust. The role includes assessment and home treatment in patients' homes within the community and/or treatment rooms at the Stonebow Unit. Services operate 24 hours a day, 7 days a week. The successful candidate will work 37.5 hours per week, over 3/4 days or nights (12‑hour shifts).
Responsibilities
The duties include being called at any time to a crisis situation, discussing referrals each morning, reviewing caseloads three times weekly with the multidisciplinary team, completing comprehensive risk assessments (independently or as part of the MDT), and liaising with families, carers, GPs, community mental health teams, social care teams, community nurses, physiotherapists, voluntary organisations and others to maximise recovery.
