The Discharge Liaison Officer is a new and exciting role supporting and facilitating the safe and timely discharge of patients. The key element of the Discharge Liaison Officer role will be to communicate with BTHFT colleagues, the MDT, patient, families, medical and nursing staff to ensure everyone is working to the same discharge plan. This list is not exhaustive and it is expected that there will be other key relationships which will develop within this role. Advert The post holder will need to be prepared to work 7.5hr shifts 5 days per week. The role will be to support the MAIDT to facilitate effective discharge. Detailed job description and main responsibilities Summary of most important Experience/Skills/Qualifications required: Excellent written and verbal communication and interpersonal skills. Competent in use of EPR and a range of IT systems. Current experience of multidisciplinary team work.
