Weldmar Specialisthospice at Home Service

30 hours per week, Permanent contract

Clinical Salary Scale: £37,804 - £46,311 per year FTE

We are looking for a Weldmar Community Nurse for our newly formed Specialist Hospice at Home Service in North Dorset.

You'll be joining Weldmar at an exciting time, working collaboratively with the multi-skilled team in our North Hub, other clinical colleagues, and external agencies to provide an agile and responsive service based on patient needs in Sturminster Newton, Sherborne, Blandford, Shaftesbury, and surrounding areas.


Main duties of the job

This new and exciting service will ensure our patients, carers, and families benefit from receiving the most appropriate care, as determined daily. They will have access to support from the hub, other clinical specialists from our multidisciplinary team, our patient and family support services, and hands-on care if required, enabling them to live as well as possible at home.

You will use your significant post-registration experience as a registered nurse or paramedic to complete ongoing assessments of our patients, ensuring the delivery of quality holistic evidence-based palliative care. You'll have time to assess complex needs and take steps to positively impact patients and their loved ones when it matters most. You will also play a key role in advising, coaching, and guiding colleagues within the hub and other health professionals to ensure outstanding end-of-life care in the community.

Your core hours will be Monday to Friday, 9 am to 5 pm, with a requirement to provide out-of-hours weekend cover for the advice line on a rota basis.

About Weldmar Hospicecare: Rated Outstanding by the CQC, Weldmar Hospicecare provides specialist care to adults in Dorset with a life-limiting illness, and support to their loved ones. This care is given in patients' homes, in the community, and at our Inpatient Unit in Dorchester. All care is free of charge and mainly funded by local community support, fundraising, our retail chain, and gifts in wills, enabling us to raise the £27,500 needed daily to sustain our services.

Job responsibilities

The role of the Weldmar Community Nurse involves working collaboratively with the Specialist Hospice at Home (SH@H) team, closely with colleagues within the hub, primary care teams, and community hospitals. The role includes providing advice and guidance to patients, carers, and other professionals to ensure high-quality, integrated services and responsiveness to patient needs.

The role encompasses clinical, advisory, therapeutic, and educational functions.

To practice evidence-based expert palliative and end-of-life care, managing a flexible caseload under the guidance of the Weldmar Community Nurse Specialist.

To provide guidance, education, mentorship, and support to colleagues within the hub and other healthcare professionals to develop and improve palliative care practice.

To act as a resource, offering advice, guidance, and support to Healthcare Assistants and Assistant Practitioners within the team.

Undertake regular line management with the service's Assistant Practitioner and Healthcare Assistant team, in line with policy and individual needs.

Participate in audits and support service and practice development.

Respond to patients' needs daily as part of the specialist hospice at home service.

Support patients and families during the end-of-life phase.

Promote the use of AIRMID/video technology.

Provide guidance and advice on holistic management of patients, working in various settings including homes and clinics.

Manage allocated patients within a corporate caseload, providing advice, guidance, and support both independently and as part of the multidisciplinary team.

Recognize and work within your own competency limits, consulting with team members as needed.

Maintain accountability following the NMC Code and organisational policies.

Undertake holistic patient assessments, discuss diagnosis, prognosis, and end-of-life issues sensitively, and empower patients through informed decision-making.

Provide emotional support, refer for additional psychological or social support, and ensure patients and families receive appropriate information.

Identify when a patient is entering the dying phase and ensure appropriate care and support.

Administer PRN medication in-home if present and the patient is distressed, as required.

Line manage and support colleagues, ensuring adherence to policies and scope of practice.

Develop and maintain relationships with patients, families, and professionals, ensuring effective communication and collaboration.

Participate in ongoing professional development, clinical supervision, and external networks.

Lead advance care planning discussions with patients about their end-of-life wishes, including resuscitation decisions.

Demonstrate conflict resolution and negotiation skills when dealing with challenging situations.

Assess for complicated grief and refer families and carers for bereavement support as needed.

Follow the duty of candour in all communications with patients and carers.
