Health and Wellbeing Nutritional Coach (HWBC)

RavensbournePCN consists of two main practices Novum Health Partnership and ICO HealthGroup. We work within the Neighbourhood 3 footprint in Lewisham.

RavensbournePCN work closely with our partners to deliver equitable healthcare to where it is needed most and this role will initially be an integral part of IntegratedNeighbourhood Team.

The key members of this partnership are Lewisham & Greenwich NHS Trust, the South London and Maudsley NHS Foundation Trust, Lewisham council, GP practices, Primary Care Networks and our GP Alliance, as well as Healthwatch Lewisham and local voluntary sector organisations and community representatives.

We are building a health and care system that's not just sustainable, but supports peoples physical and mental wellbeing. By putting peoples needs at the centre of everything we do, we're ensuring access to high-quality, tailored support when it's needed most.


Main duties of the job

Lewisham is transforming neighbourhood care through the establishment of Integrated Neighbourhood Teams, designed to deliver coordinated, community-based care and support. The teams are made up of NHS services, primary care, social care, and the voluntary sector to provide more integrated and effective support for residents.

The four INTs align with Lewisham's neighbourhoods and are closely integrated with seven Primary Care Networks. This approach builds on existing health and care service boundaries and proven models of successful collaboration in adult social care, children and young people services, district nursing, and mental health services.

To ensure a controlled and effective rollout, the initial focus of INTs will be on supporting patients with three or more cardiovascular long‑term conditions.

Residents accessing the service will receive holistic support that addresses both physical and social factors affecting their lives. This includes:

* A comprehensive multi‑morbidity assessment to understand and manage long‑term health conditions
* Access to the Lifestyle Medicine Service, offering personalised support for healthier living
* Group consultations to share experiences, learn from others, and receive expert advice
* Support with social challenges, such as housing, finances, or loneliness because wellbeing goes beyond just health


About us

Manage and prioritise a caseload based on individual needs and priorities, patient referrals and waiting lists. Adapt service delivery to meet the diverse needs of the local population, ensuring accessibility and inclusivity. Work flexibly to meet evolving service and patient demands. Deliver one-to‑one and group health and nutritional coaching sessions incorporating all six pillars of lifestyle medicine. Provide personalised nutritional coaching aligned with patients goals and health challenges, supporting behaviour change and self‑management through structured coaching techniques.

Raise awareness of the Lifestyle Medicine Service, particularly among underserved communities. Collaborate within multidisciplinary and multi‑agency teams to promote health coaching and personalised care. Build strong relationships with GP practices, PCN teams, and community services to ensure effective referrals and continuity of care.

Act as Lead Health and Wellbeing Coach, supporting colleagues and championing lifestyle medicine principles across the PCN. Contribute to team, practice, and network meetings and service development. Provide leadership on nutrition‑focused interventions.

Collect and analyse feedback and data to evaluate and improve service delivery. Contribute to reporting and performance evaluation. Engage in supervision and CPD to enhance communication, coaching, and service quality.

The post‑holder may be required to work at any appropriate site within the neighbourhood as necessary for the delivery of the functions of this role.

Records Management

You have a legal responsibility for all records you work with e.g. patient records, financial records, personal, administrative, etc that you gather or use as part of your work. The records may be held in a variety of formats such as paper, electronic, microfiche, audio, and video tapes, etc. You must consult your manager if you have any doubt as to the correct management of the records with which you work.

Confidentiality

In the course of your employment, you will have access to confidential information. You are required to exercise due consideration in the way you use such information and should not act in any way which might be prejudicial to the organisation's interests. If you are in any doubt regarding the use of information in the pursuit of your duties, you should seek advice from your Line Manager before communicating such information to any third party. Any inappropriate disclosure may be subject to disciplinary procedures.

Infection Control

All staff are responsible for protecting themselves and others against infection risks. All staff, regardless of whether clinical or not, are expected to comply with current infection control policies and procedures and to report any problems regarding this to their managers.


Person Specification


Experience

* Experience working in a team environment
* Experience in health and wellbeing coaching
* Experience working as a nutritionist
* Experience delivering lifestyle medicine approaches with patients
* Experience of working with patients living with pre‑diabetes and or diabetes
* Experience in healthcare, social care, or community development settings
* Experience working in multidisciplinary teams
* Previous experience in the NHS or a similar healthcare setting
* Familiarity with clinical systems such as EMIS
* Experience with working with those with Learning Disabilities and SMI


Qualifications

* Minimum of five GCSEs / O Levels or equivalent
* Demonstrable commitment to ongoing professional development PCI‑accredited coaching qualification
* Willingness to undertake further training
* Evidence of continued learning and development
* Foundation Course or IBLM Diploma in Lifestyle Medicine


Skills, Abilities and Other

* Excellent communication skills including written, verbal and use of IT.
* Motivational Interviewing and coaching skills
* Ability to analyse and interpret information about health and well‑being
* Ability to undertake needs assessment activity with communities
* Ability to communicate with a wide range of people about health and well‑being
* Ability to apply evidence‑base knowledge into practice
* Ability to develop and maintain effective collaborative working
* Ability to work in partnership with individuals and communities to improve health
* Ability to plan, implement and review projects and work streams; ability to organize work under pressure and use your own initiative
* Can speak multiple languages

£34,000 to £40,000 a year – depending on experience. WTE / FTE
