* Location: Shepton Mallet, Somerset
* Pay Rate: £38 per hour
* Working Pattern: Hybrid – Minimum 2 days per week in the office
* Contract: Full Time
About the Role:
We are seeking an experienced and motivated Social Worker to join the Acute Hospitals Adult Services Team. This is an excellent opportunity for a skilled practitioner with experience in hospital-based social work and multi-disciplinary working, supporting adults through safe and effective discharge planning and ongoing care arrangements.
Key Responsibilities:
* Complete strengths-based assessments and care planning for adults within an acute hospital setting.
* Facilitate safe, timely, and person-centred hospital discharges.
* Work collaboratively within a multi-disciplinary team, including healthcare professionals and community partners.
* Assess eligibility for services and support in line with relevant legislation and local procedures.
* Promote independence, wellbeing, and choice through effective intervention and support planning.
* Participate in safeguarding enquiries and risk management processes where required.
* Maintain accurate, timely, and comprehensive case records.
