Job Title: Consultant for Hospital at Home
Dates: Commencing ASAP for 6 months initially with view to extend
Rota: 9 am to 5 pm, Monday to Friday. Involves providing comprehensive assessments and management plans for frail elderly patients in both acute and community settings.
