About The Role

At The Forward Trust, we are committed to supporting individuals in their recovery from substance misuse and mental health challenges. We’re currently recruiting a Health & Wellbeing Practitioner to join our dedicated team at HMP Coldingley, a closed Category C male prison in Surrey.
As part of an integrated healthcare framework, you’ll work alongside a multi-disciplinary team to deliver a mix of clinical and psychosocial interventions to adult male prisoners, including individuals presenting with low-level mental health needs and/or substance misuse issues.
You Will Be Responsible For
* Managing a caseload and delivering tailored recovery-focused interventions
* Conducting assessments and co-producing SMART care plans with service users
* Facilitating structured 1:1 sessions and group programmes
* Providing harm reduction advice and practical recovery support
* Supporting release planning and ensuring through-care into community services
* Performing second signatory duties as needed
* Maintaining accurate, up-to-date case records in line with service KPIs
This is a 7-day-a-week service, so weekend working will be required on a rota basis.

* Confident using digital tools, case management systems, and able to maintain accurate, timely records.

* Thrives in fast-paced, changing environments, with the ability to respond flexibly to service demands.
