We would like to recruit an experienced pharmacist who can prescribe, but we will consider applications from pharmacists who are in the process of completing their prescribing training or are willing to undertake a prescribing course.

This vacancy is 30 hours per week, over five days, Monday to Friday, 9am until 3:30pm with a 30-minute unpaid break.


Main duties of the job

* Support the practice in all aspects of medicines management, facilitate cost-effectiveness and safe prescribing.
* Undertake medication reviews with patients.
* Handle prescribing queries from clinicians and review patients' clinical conditions, including diagnosis, therapeutic monitoring, and prescribing as agreed with clinicians.
* Liaise with hospital and community pharmacists and other healthcare professionals.
* Reconcile medicines following hospital discharge and work with patients and community pharmacists.
* Support patients in the effective use of medicines through medication review services.
* Advise the practice on safe, efficient repeat prescribing systems to reduce waste and improve efficiency.
* Act as a medicines information resource for the team and patients.
* Support the practice in adapting prescribing practices to local and national policies and targets.
* Liaise with nursing homes on current issues.


About us

We are a forward-thinking 6-partner, 4-salaried GP practice based in Kippax and Garforth, with a dispensing branch in Monk Fryston. Our list size is approximately 11,200.

We seek an enthusiastic pharmacist to join our team. We have established systems for managing discharge letters, medication reviews, and prescribing issues, with support from our local PCN pharmacist team. Our facilities include modern buildings and collaborations with PCN and community staff such as First Contact Physiotherapists, Physician Associates, Chronic Disease Nurses, and Social Prescribers.


Job responsibilities

* Support medication management, including medication reviews via various modes of communication.
* Handle prescribing queries, including reviewing clinical conditions and using clinical systems.
* Liaise with healthcare professionals to ensure seamless patient care and promote EPS and repeat dispensing.
* Reconcile medicines post-discharge and collaborate with patients and pharmacists.
* Provide medication reviews to support effective medicine use.
* Advise on safe, waste-reducing repeat prescribing systems.
* Serve as a source of medicines information for the team and patients.
* Assist in implementing prescribing strategies aligned with policies and guidelines.
* Manage own time effectively, planning workloads and projects.
* Engage in continuous professional development and training.
* Liaise with local pharmacies and nursing homes on current issues.


Person Specification


Knowledge and skills

* Excellent communication skills
* Ability to work independently and as part of a team
* Motivated to support high-quality medicine management
* Understanding of confidentiality and professional policies
* Experience analyzing ePACT data
* Adaptability to different environments
* Professional presentation and recognition of personal limitations
* Understanding of read coding and summarizing processes
* Good report writing and presentation skills
* Ability to motivate change in others


Qualifications

* Degree in Pharmacy
* Registration with the GPhC
* Postgraduate pharmacy qualification
* Proficiency in office applications, internet, email, and electronic diaries
* Prescribing qualification or working towards it
* Experience in secondary care and discharge processes
* Experience in primary care/PCN settings
* Minimum of 2 years postgraduate experience in pharmacy


Experience

* Up-to-date pharmaceutical knowledge
* Experience in multidisciplinary environments
* Data analysis skills
* Time management and prioritization skills
* Experience working within primary care systems


Disclosure and Barring Service Check

This position requires a DBS check in accordance with the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975, to ensure suitability for working with vulnerable groups.
