Overview
Join Our Team as a Care Co-Ordinator and Make a Real Difference Every Day! Our approach to healthcare is refreshing, innovative, and rooted in care. We put patients at the heart of everything we do, and we’re looking for someone passionate to join us on that journey.
As a Care Co-Ordinator, you’ll play a key role in supporting patients through every step of their care journey. Working across Amesbury and nearby communities, you’ll become a vital point of contact for patients and their carers, someone they can rely on for guidance, reassurance, and support.
You’ll work closely with GPs and other healthcare professionals within the Primary Care Network to identify and support patients who would benefit from extra coordination and care planning. By helping them navigate the healthcare system with ease, you’ll ensure their needs are met, their care is well joined-up, and their wellbeing is always the top priority.
This is a role where your compassion, organisation, and communication skills will truly shine – and where you’ll know you’re making a real impact, every single day.
If you’re ready to be part of a friendly, values-driven team that’s committed to better patient care, we’d love to hear from you.

We deliver and transform adult and children community health services, primary care services including urgent care, sexual health, dermatology and MSK services as well as adult social care and wellbeing services. Across England, we support communities of many millions and directly help more than half a million people each year - guided by our simple values: we care, we think, we do.
We’re committed to equal opportunities and welcome applications from a broad, diverse range of people who want to join our team. We’re a Disability Confident Committed company, so we work to provide facilities, work environment adjustments and technical solutions to be as inclusive of everyone.
