Are you looking to progress you clinical and leadership skills within an Integrated Mental Health Team?

The Isle of Dogs Neighborhood Community Mental Health Team is part of the South East Locality catchment are of the London Borough of Tower Hamlets which covers Primary Care Network 7 and 8. We are an established Mental Health team with motivated, proactive and supportive team members. We care for service users with complex needs, including severe and enduring mental health needs requiring us to provide skilled assessment and interventions through the integrated Health and Social Care model of care.

This is an exciting opportunity for the successful applicant to develop their understanding and delivery of Health and Social Care. The successful applicant will work alongside a multi-Disciplinary team and will co-locate with partner organisations to ensure that provision of health and social care is delivered in a coherent and integrated pathway.

The Team promotes opportunities for professional development through regular supervision, access to training, including managerial and leadership and quality improvement (QI) projects.

Main duties of the job

The post holder will be responsible for:

1. Work closely with PCNs/GPs, Social Services, Housing and other statutory and voluntary agencies to navigate care pathways and ensure that service users benefit from person-centered care and support plan.
2. Actively engage service users and their carers in all stages of care provision, ensuring that their views and what matters to them are acknowledged and taken into account.
3. Be part of the Duty Rota for the Team, in line with responding to queries as they arise.
4. Undertake preparation and producing of statutory and other reports, and conducting other delegated responsibilities on behalf of the Team.
5. Have the responsible for the administration of depot medication and other clinical interventions.
6. Participate in quality improvement initiatives and projects, audits and surveys relevant to the Service.
7. Be part of Continues improvement to ensure quality of service delivery is maintained.
8. To proactively engage in supervision and appraisal and provide supervision to junior colleagues.

Detailed job description and main responsibilities

· To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.

· To promote a service user focused service.

· Participate in audits and survey relevant to the team.

· To participate in development of new policies and procedures as required.

· Ensure that clients receive a culturally competent service.

· To co-ordinate the day-to-day activity and manage the workload of the Team.

· To ensure the legal requirements of the Mental Health Act (1983) and subsequent amendments are adhered to.

· To establish and maintain good relationships with service users, carers and voluntary sector colleagues.

· To provide best evidence practice to service users which complies with legislation, policies and procedures and relevant quality, governance and local and national guidelines.

· Liaise with General Practice and Primary Care colleagues to promote partnership working.

· Ensuring that the local demographics are taken into consideration for service provision and improvement

Education/ Qualification/ Training

Essential criteria

9. Registered Mental Health or Registered Learning Disabilities Nurse
10. Evidence of continual professional Development
11. Trained Adult Safeguarding Enquiry officer or willing to undertake Enquiry Officer training.

Desirable criteria

12. Higher Education qualificatio n in Mental Health Nursing AMHP or willing to undertake AMHP training

Experience

Essential criteria

13. Experience in a relevant field in Mental Health
14. Experience in working with a diverse community in a complex environment
15. Experience of working as part of a multi-Professional or multi agency team

Desirable criteria

16. Experience of working with service user (and carers) affected by cognitive impairme

Knowledge

Essential criteria

17. Understanding of the Mental Health Act 1983 and how the act is used in practice.
18. Understanding of the Mental Capacity Act (2005) and how the act is used to support service users and carers / relatives.
19. Knowledge and understanding of the Care Act 2014 and how the is Act implemented within care provision.
20. Knowledge and understanding of approaches to risk assessment and management.

Skills

Essential criteria

21. Ability to communicate effectively both verbally and in writing.
22. Ability to prioritise workload.
23. Excellent time management skills
24. Ability to work autonomously and independently
25. Good IT skills.
