Your role –
8. GPwER Frailty
9. Holding a list of elderly and frail patients only 30-minute F2F appointments in-practice 1 hour – 1.5 hour home visits conducting comprehensive care evaluations Care home oversight, rounds and reviews
10. Working collaboratively with wider team including Clinical Pharmacists, Pharmacy Technicians, Paramedics, Frailty Practitioners, Physiotherapists, Occupational Therapists and Chronic Disease Nurses
11. Participating in and often leading MDT meetings
12. Up to 10 sessions available with a minimum of 4 sessions required
