Senior Manager – Community Response Team
An exciting opportunity has arisen for an experienced, proactive, and passionate Senior Manager to join our innovative Community Response Team. This is a pivotal leadership role within a forward‑thinking service transforming how care is delivered—supporting patients to remain safely at home and reducing avoidable hospital admissions.
Responsibilities
* Shape and deliver the vision for community care, ensuring patients can remain safely at home and reducing avoidable hospital admissions.
* Lead the Community Response Team, working in partnership with local authorities, primary care, NEAS, and voluntary sector to achieve the NHS 10‑Year Plan.
* Collaborate across organisational boundaries, linking seamlessly with external partners and acute teams.
* Champion equitable, high‑quality care across diverse communities, from rural areas to those with significant health inequalities.
* Lead teams through personal example, fostering a culture of diversity, equality, innovation, customer focus, and performance.
* Use coaching to promote ownership of learning and quality improvement, facilitating team working and collaboration across teams and departments.
* Implement best practice and value‑for‑money solutions within the specialist area to support the trust’s strategic vision.
Qualifications
* Experience as an inclusive and collaborative leader in community services.
* Proven ability to work across organisational boundaries and lead change initiatives.
* Strong understanding of community health needs and how to deliver patient‑centred care.
* Knowledge of NHS strategic plans (e.g., NHS 10‑Year Plan) and experience working with local authorities, primary care, NEAS and voluntary sector.
* Demonstrated ability to drive performance culture and use coaching for learning and improvement.
