Job Summary
The Clissold Park PCN in City and Hackney is seeking to recruit an experienced PCN Clinical Pharmacist to develop and support medicines optimisation work within the GP practices in the PCN covering a population of approximately 37,000.
Job Responsibilities
* Join a core Pharmacy Team and be supported by a Senior Clinical Pharmacist and a GP Supervisor
* Work with another pharmacist with experience in practices to support
* Be welcomed to undertake the independent prescribing course with us (if already completed CPPE or nearing completion); regular development sessions, teaching, training and opportunities to upskill will be provided
* Have support via monthly group meetings with the senior pharmacist
We have a well-structured pharmacy team in Clissold Park PCN, currently comprising 5 pharmacists and 2 pharmacy technicians across the PCN. Clinical Pharmacists support the delivery of the new network contract Directly Enhanced Service (DES) specifications including Extended Access. The PCN Clinical Pharmacist will need to be committed to improving health and putting patients first. Key qualities include the ability to build relationships and work collaboratively with the practice team and build rapport with patients. Demonstrable general pharmaceutical skills and knowledge in core areas, including long term conditions, and a good understanding of general practice, are essential.

Groups of GP Practices in City and Hackney have come together to form 8 Primary Care Networks (PCNs), each covering a total population of between 30,000-56,000. The purpose of these PCNs is to enable GP practices to work together in a collaborative way to develop and deliver network-based services that respond to the needs of the local population.

Job Description
Job responsibilities include: working as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using medicines expertise; pursuing prescribing status or training; managing patients with long-term conditions and undertaking clinical medication reviews, especially in polypharmacy and frailty; providing specialist medicines expertise and leading medicines optimisation systems and processes; contributing to QoF, enhanced services and IIF indicators; and supporting integration with wider healthcare teams.
Key Aspects of PCN Pharmacist Role
* Patient-facing face-to-face medicines reviews for patients with long-term conditions and polypharmacy, housebound patients, and priority chronic disease management areas
* Review ongoing need for each medicine, assess monitoring requirements and support safe, appropriate, optimised treatment
* Signpost patients to self-care and community pharmacy services; improve adherence
* Medicines information to practice staff and patients; provide updates at clinical meetings; analyse prescribing data
* Medicines reconciliation on discharge from hospital or care settings; ensure seamless transfer of care
* Support QoP/IP/benefits improvements, NRLS reporting, and shared care protocols
* Signposting, repeat prescribing management, and monitoring, including safety alerts
Knowledge, Skills and Experience
Essential: experience with common acute and long-term conditions in general practice; minimum of 2 years post-graduate experience in pharmacy; independent prescribing qualification (or working towards); understanding of the role of a clinical pharmacist within primary care; ability to follow professional policies and codes of conduct.
