Job overview

The purpose of this post is to actively manage the administrative processes of all patient pathways, ensuring a smooth and efficient service for patients referred to the Hospital at Home service, coordinating all Hospital at Home ward wounds and MDT meetings, supporting the multidisciplinary team, and providing an effective and efficient administration service across all patient and digital pathways.

Working as part of a job share across a 7 day per week rota, the post holder will be responsible for the facilitation and coordination of multi-disciplinary team meetings ensuring MDT meetings run effectively and are well structured and communicated, focusing on customer service and team working.

Main duties of the job

1. To manage the process, content and output of the MDT/Planning meetings utilising guidelines created and agreed by the specialities to locally and nationally agreed standards.
2. Monitor and maintain the databases supporting patient information in the specialities, take responsibility for ensuring the quality of information recorded.
3. To work as part of a team to ensure cross cover for MDTs and planning.
4. Supporting all digital pathways as a digital champion into services across the single point of access

9. NVQ- L3 in administration or equivalent demonstrable experience
10. GCSE Maths and English (A-C) or equivalent experience

Knowledge and Skills

11. Evidence of continued learning and development
12. To be proficient in Microsoft Office programmes including Word, Excel and Outlook
13. Experience of producing reports from databases
14. Experience of setting up spreadsheets to record a data for reports
15. Understanding of medical terminology
16. Experience of using SystmOne or similar digital patient systems
