A Social Work opportunity has arisen within the Hospital Discharge Service of the Community Resource Team (CRT).
We are a large multi-disciplinary team, working at pace to ensure timely, safe discharges for the residents of our community. The post holder will need to cover several hospitals both in Caerphilly and neighbouring Boroughs, and some experience of hospital discharge work would be an advantage.
You will be part of a forward-thinking evolving team and will be supported by experienced colleagues and Management Team.
For the role, we ask that you have:
A Diploma / Degree in Social Work or predecessor qualification.
The ability to construct care plans to demonstrate the required outcomes, minimising risk and meeting individual need.
Experience of work within a hospital/community setting.
