Job Description:
Position: Manager, Clinical Programs on the CKD Model of Care team at DaVita will contribute to providing integrated kidney care (IKC) to patients with chronic kidney disease (CKD). The role involves developing and implementing strategies to achieve TCOC reduction and optimal patient outcomes.
The candidate should have a driven attitude, strong organizational skills, excellent communication, and the ability to build relationships within a dynamic, cross-functional team environment.

Education & Experience:
* Bachelor’s degree required
* 4+ years healthcare experience required
* Experience managing direct reports preferred
* Collaborative with change impact skills
* Experience with healthcare initiatives and pilot programs preferred
* Excellent communication skills

Salary range: $75,000 - $110,000 per year. Compensation depends on qualifications and experience.
