About the Role Ludham & Stalham Green Surgeries has a fantastic opportunity for an advanced nurse practitioner to join our friendly team, on a part-time basis of 20 hours per week, although hours can be flexible. We are delighted to offer a “Golden Hello” payment to the successful candidate and the possibility of Fellowship opportunities. The ANP will share the workload & responsibility with GPs by assessing patients with undifferentiated & undiagnosed problems, making assessment of their needs, implementing, evaluating & prescribing programmes of care & referring as appropriate. The main responsibilities of the role include:
1. Ensuring access & delivery of primary care clinical services to the practice population.
2. Assessing, diagnosing, planning treatment & care, implementing, prescribing, referring, & evaluating programmes of care
3. Participating in clinical supervision & actively participate in meetings
4. Providing professional & clinical advice
5. Promoting evidence-based practice
6. Participating in teaching & mentoring nursing students & other health professionals & nurse practitioner degree students
7. Being aware of & ensuring clinical governance & risk management for the practice.
8. Actively promoting & supporting development of integrated working in collaboration with other primary care professionals
9. Ensuring effective liaison with other health professionals, services & agencies to enable the delivery of integrated patient focused service
Please see full job description attached. About the Candidate All applicants should have the following qualifications, registrations, skills and experience:
10. A relevant nursing degree.
11. Current NMC registration with an active PIN.
12. Recognised Nurse Practitioner qualification or equivalent training.
13. Independent prescribing qualification.
14. Evidence of CPD.
15. Nurse revalidation.
16. Time management including the ability to work without direct supervision and determine own workload priorities.
17. Ability to work under pressure.
18. Ability to identify, work through and resolve problems.
19. Ability to recognise matters of confidential nature and respect as such.
20. Ability to work within and adapt to a changing environment
21. Ability to work between both sites and provide home visits as required
22. Proactive ‘can do’ attitude.
23. Safe and effective clinical practice remaining within boundaries of competence and limitations.
24. Knowledge and application of chronic disease management.
25. Ability to diagnose and treat minor illness recognising when referral to another professional is necessary
26. Competent IT and computer skills.
27. Ability to communicate at all levels.
28. Excellent team working skills, working as part of a small team within a large team.
