In this role, you will work under the guidance of qualified practitioners, holding a caseload and carrying out assessments to develop tailored support plans. You will play an active role in risk management, monitoring outcomes, and ensuring individuals receive the most effective support.
Who we are looking for…
We are looking for a compassionate and motivated individual with experience or awareness of social care, healthcare, or the voluntary sector. You should have a Level 3 qualification in Care and strong communication skills to collaborate with customers, carers, and stakeholders.
