The position of Social Worker at Elysium Healthcare in Newark involves working full‑time on a fixed 18‑month contract. The role is pivotal in impacting lives positively by aiding vulnerable individuals towards a brighter future. The job emphasizes teamwork, as you’ll be collaborating with a dedicated team in the Farndon Unit, a secure hospital with personalised treatment pathways for women with mental health and complex care needs. You will manage a complex caseload and perform tasks such as social care assessments, risk evaluations, and facilitate group activities, alongside participating in various meetings to support the service users future planning.
Job responsibilities
As a Social Worker, you will be responsible for:
* Referrals to LADO and Local Authority in line with safeguarding and assessments
* Educating hospital staff about relevant safeguarding matters
* Completing a full annual social care reassessment within the CPA format
* Facilitating access to welfare rights, housing, advocacy services and associated support services
* Participating in 1:1 supervision
* Managing a complex caseload, undertaking social care assessments, risk assessments, and facilitating group activities
* Contributing to ICR meetings and supporting service users to make viable plans for their future
* Reporting to the Safeguarding Lead, managing internal investigations, and liaising with home authorities during admissions and discharges
Qualifications
* Registered with SW England
* Social Work degree
* Experience within a similar environment working with adults
