 
        
        Job Summary
Our Primary Care Liaison Team is the front door connecting people with the right support at the right time. We provide initial assessments and support conversations for people who may require secondary mental health services, working closely with GPs, voluntary sector organisations and primary and secondary care to ensure people receive tailored, compassionate, and effective care.

Key Responsibilities
 * Complete initial assessments and support conversations for people referred to the service.
 * Manage urgent and routine referrals and formulate safe, effective, person‑centred care plans to connect people with appropriate support.
 * Make independent clinical decisions while ensuring robust risk management.
 * Liaise closely with primary care, secondary mental health services and voluntary sector organisations.
 * Use standard assessment tools and maintain accurate IT and documentation records.
Requirements
 * Strong clinical judgement & ability to make independent decisions in a fast‑paced environment.
 * Experience working autonomously while ensuring patient safety and effective care.
 * Proficiency in risk assessment and management.
 * Excellent communication skills to engage with people compassionately and collaboratively.
 * Empathy, resilience and a solutions‑focused approach to supporting people with mental health needs.
 * Strong IT and documentation skills.
Essential Qualifications
 * Professional qualification in mental health (e.g., Registered Nurse, Registered Social Worker or Registered Therapist).
 * Detailed working knowledge of secondary specialist mental health services and the wider health and social care community.
 * Significant experience in educating and training the mainstream workforce, particularly non‑specialist mental health workers.
 * Substantial experience of assessing risk and developing risk management strategies.
Desirable Qualifications
 * Relevant post‑registration qualifications such as CBT, AMHP, Brief Interventions, etc.
 * Experience gained in a wide variety of settings.

Green Lane Hospital, Marshall Road, Devizes, SN10 5DS. Full‑time, flexible working, compressed hours.
Salary & Contract
Band 6 – £38,682 to £46,580 per year. Permanent contract.
