Join Our Reablement Team as a Care and Support Worker
Are you compassionate, reliable, and committed to making a difference? We are seeking Care and Support Workers to join our Reablement Service, supporting individuals to regain independence and confidence in their own homes.
About the Role
1. Contract: 26.25 hours per week
2. Service: Operates 7 days a week, year-round
3. Location: Community-based care in service users’ homes
4. Purpose: Support recovery following illness, injury, or hospital discharge
Working Pattern
5. 4-week rolling rota
6. Morning shifts: 7:00am – 3:00pm
7. Includes weekends and bank holidays
8. A full driving licence and access to a vehicle are essential
About the Service
Our short-term, person-centred service supports individuals with disabilities or those recovering from illness. We focus on:
9. Promoting independence
10. Relearning daily living skills
11. Providing dignified, respectful care
12. Working collaboratively with health and social care professionals
Additional support includes:
13. Bridging care while long-term arrangements are made
14. Resettlement support following time away from home
15. Emergency response during urgent situations
Training and Qualifications
Essential:
16. GCSE Grades D–G or Functional Skills Level 1 in Maths and English
Desirable:
17. Level 2 in Health and Social Care (or equivalent)
